Disabled facilities adaptations

LWM delivers the Disabled Facilities Grant service on behalf of Lichfield District Council, helping residents access home adaptations that support safer, more independent living. Our team manages the full process from assessment through to completed works, providing practical support tailored to individual needs. 

We help residents maintain independence, improve quality of life and continue living safely in the homes they know and love. 

Our expert team provides a fully managed adaptations service, supporting clients from assessment and design through to funding, procurement and project delivery.

We deliver a wide range of adaptations, including stairlifts, wet rooms, hoists, ramps and specialist equipment, helping people remain safe, independent and comfortable at home.

Our award-winning approach was recognised nationally when we were named Foundations Adaptations Service of the Year 2025 at the National Healthy Housing Awards.

What the resident needed

The resident required adaptations to improve accessibility within the home, particularly around bathing and personal care. Difficulties with mobility were making everyday tasks increasingly challenging and reducing independence, creating a need for a safer and more accessible environment.

How it helped
The completed adaptations transformed the resident's ability to move around their home safely and independently.
